Your 360 Feedback Form will contain a series of Competence Headings e.g. "COMMUNICATION" followed by a individual behaviours which are used as the assessment criteria e.g. "Listens carefully to feedback from others."
The online form will then be used by respondents to assess the individual's performance against these criteria.
You can use forms already designed and tested by RMS or you can create your own form from scratch. There are no limits to the number of issues which can be covered but the typical range is 40-60 in total.
Once you have designed your 360° Feedback Form you are ready to collect feedback for participants.
The project administrator creates a list of those staff who are to receive feedback. These staff are then emailed a password and instructions so that they can nominate colleagues to provide feedback for them.
When the nomination phase is completed each nominated rater is emailed the site address together with their individual password. They can then click on the address and enter feedback for their colleague.

Once the feedback process is started both the project administrator and each individual can monitor the progress of each of their nominated staff using a simple on-line report.
If some of your participants do not have internet access we can supply paper based forms for them to complete their feedback. These can then be entered on to the site by the project administrator.
