Designing your 360° assessment form

You may like to include some or all of the following information on your 360° assessment form.

Competence Headings

The form is divided in to a number of competence or subject headings. You have complete control over the number and names of the headings.

Individual Behaviours

The main part of the 360° assessment form asks staff to rate their colleagues against a series of typical behaviours. You can specify the exact number and wording of these behaviours.

Rating Scales

The rating scales are used to collect the bulk of the 360° feedback. You can set the rating scales used to collect opinions, for example, a performance scale of 1 to 5. You can even set two 360° scales to be used, such as importance 1 to 5 and performance 1 to 5.

Comment Questions

As well as collecting 360° feedback as ratings, you can also define free text responses. These are collected under specific headings called ‘comment questions’. Such a question might be: “What are this individual's main strengths?”

Reporting Relationships

Choose which relationships you wish to report under, such as: self, manager, colleague, direct report. When participants review their 360° feedback they can see it divided under these relationship headings.
